I think he wears briefs. I can’t remember. I haven’t watched the Simpsons since I was younge. But I remember him pulling his pants down for a spanking and he was wearing briefs. It was the Itchy and Scratchy episoid where he was grounded from going to the movies.

I remember he wears briefs. There are several episodes in which he runs around the house in his underwear. I haven’t watched the movie yet, so I don’t know about that.
